Amica’s Participation in the Massachusetts Drive Program
Amica invites auto dealerships participating in the Massachusetts Drive Program to contact us, either by phone, fax or email, to report new or used car purchases on behalf of their customers.

Drive Program requirements
- If a new car was purchased:
- Copy of the front and back of certificate of origin
- Copy of the window sticker
- Completed RTA form
- If a used car was purchased:
- Copy of the front and back of title
- Completed RTA form
Additional information needed
- Along with the RTA form, send us the following required information:
- Customer’s name
- Dealership’s name
- Dealership’s fax number
- Amica’s policy number (optional)
