How to Report a Death Claim

The loss of a loved one is hard enough. Here you’ll find clear guidance on how to file an Amica life insurance claim and what documents you may need.

After a death has occurred

  • A close family member should request five to 10 certified copies of the death certificate from the funeral director. Each life insurance company may request a certified copy of the death certificate. Your bank and Social Security Administration may also need this document.
  • If family members are not sure where the deceased may have had life insurance policies, review the checkbook of the deceased to determine if any checks were written to insurance companies. In addition, review credit card statements to determine if any recurring debits are made to insurance companies.
  • Call each insurance company to report the claim. You will be carefully guided through the steps needed to settle a life insurance claim.

  • Any life insurance or annuity policy numbers, if available 
  • Telephone numbers of the insurance companies 
  • Their will 
  • Any financial instructions 
  • Certain claims may require medical history details including doctors’ names and dates of visits

The beneficiary or beneficiary's representative can file a death claim in one of two ways: They can call 800-234-5433, ext. 89079, to speak with Life Claims, or they can report the claim online.

You’ll need information regarding the manner and cause of death, and typically a certified copy of the death certificate. In some instances, we may need to obtain medical history information, including doctors’ names and medical treatments. An Amica representative is always available to assist and make this process as easy as possible.
