Drive program requirements

To help us process your customer’s purchase, please provide the following documents.

For a new car purchase:

• Copy of the front and back of Certificate of Origin
• Completed RTA form

For a used car purchase:

• Copy of the front and back of title
• Completed RTA form

Additional information needed:

• Customer’s name
• Dealership’s name
• Dealership’s fax number
• Amica’s policy number (optional)
